What one of the human-side approval doors did — Session.approveAsk, Session.declineAsk, Session.revokeAsk, Session.alwaysApprove, Session.revokeAlwaysApprove.

A typed REFUSAL rather than a throw, because these run inside click handlers: a button that throws takes the page down, while a button that reports gets to show the person what went wrong.

• 'UNKNOWN_ASK' — no such ask (or no such standing grant).
• 'ASK_ALREADY_ANSWERED' — a decision is already recorded; nothing here ever overwrites one.
• 'ASK_ALREADY_SPENT' — the yes was already spent by a fire (revokeAsk): revoking cannot un-fire the past, and the journal keeps the 'used' row where an auditor can count it.
• 'REVOKE_UNANSWERED' — nothing to withdraw: the person has not decided. To answer no, Session.declineAsk is the right verb; revoke exists for taking back a yes already given.
• 'WRONG_PRINCIPAL' — only the human side withdraws a human decision (revokeAsk): a caller that names a non-'user' principal is refused, in either direction.
• 'NEEDS_DECIDER' — by is required: an approval whose decider is unknown is the very claim-as-fact this closes.
• 'NOT_ENFORCED' — the session was not created with requireHumanApproval, so this row would authorize nothing.
